| 1. Make your regularly scheduled appointment and arrive on time. -- 1 Nickel 2. Your tooth brushing is GREAT or greatly improved. -- 1 Nickel 3. You have no lost, broken or loose brackets, wires or appliances. -- 1 Nickel 4. You wear your appliance as instructed. (Headgear, rubber bands, lip bumpers, etc.) -- 1 Nickel 5. Wear your T-shirt to your appointment. -- 1 Nickel 6. Bring us your report card. Make good grades or improve your grades. -- 5 Nickels 7. Refer a patient to our office. Be sure they tell us YOU referred them to our office! -- 10 Nickels 8. The New Patient that you referred to us starts active treatment with appliances or brackets. -- 20 Nickels 9. Visit your General Dentist for your regular 6-month cleaning and bring us a receipt or note from your dentist. -- 5 Nickels 10. If Dr. Middleton or any of the Staff see YOU outside of the office with your t-shirt on: like at the mall, movies, baseball or football games, etc. -- 30 Nickels |
